INSURANCE

Prior to your admission, as part of the pre-admission process, you will be asked to sign an "Assignment of Benefits" form which will be filed after the procedure and will allow payment to be sent to the ASC. If the insurance company sends payment to your home, it is your responsibility to send the payment to the ASC.

It is your right as a patient to request information or have questions answered regarding fee or payment policies. If you have any questions regarding your bill, please contact our Finance Department between the hours of 8:30 a.m. and 4:30 p.m. Monday to Friday at 718-515-3500, extension 3152.
